Affidavits (Form 4D) are required by the Rules of Civil Procedure (Ontario) for civil motions. Affidavits are used to present facts and evidence to the court for motion hearings. See Rule 37.10(2). LINK: https://canlii.ca/t/555jm
The court provides forms here for free. LINK: http://ontariocourtforms.on.ca/en/rules-of-civil-procedure-forms/
However, these forms are not formatted according to the Rules of Civil Procedure. There are also no directions on how to fill them out. Litigation Help now offers templates to make completing court forms easier. Be sure to also check out our Guide for Affidavits.
